How many hours a day should I study?

Should every subject really get equal time?

Equal time is a fair starting point, but shift extra hours toward subjects where your grades are weakest or the exam covers more material.

Does this include breaks?

No — the hours are study hours. A good rhythm is 25–50 minutes of study followed by a 5–10 minute break, which keeps focus high across the whole session.
